5E D&D Lovecraft-ian Horror: Mordiggian Servitor, Ghoul

This is the latest entry in a series of creatures I converted for Fifth Edition Dungeons & Dragons from Lovecraftian fiction. You can download all 100 monsters here 

Ghouls are an Underdark-dwelling race who feast on the dead. They have canine-like features and a rubbery flexibility to their limbs. They can speak other languages, but their own language is a bizarre tongue described as "gibbering" and "meeping." 

Half-Ghoul

Ghoulishness is a state of mind; humans become ghouls through an unknown process, led by their priests. Half-ghouls are smarter than their ghoul brethren and often rise to positions of leadership, acting as intermediaries between ghouls and races who are uncomfortable with their disgusting habits. 

Ghoul

Keen Smell. The ghoul has advantage on Wisdom   (Perception) checks that rely on smell.

Pack Tactics. The ghoul has advantage on an   attack roll against a creature if at least one of the ghoul’s allies is   within 5 feet of the creature and the ally isn't incapacitated.

Rampage. When the ghoul reduces a creature   to 0 hit points with a melee attack on its turn, the ghoul can take a bonus   action to move up to half its speed and make a bite attack.

ACTIONS

Multiattack. The ghoul makes three   melee attacks: one with its bite and one with its tombstone, or one with its   bite and two with its claws.

  • Bite. Melee Weapon Attack: +5 to   hit, reach 5 ft., one creature. Hit: 6 (1d6 + 3) piercing damage. 
  • Claws. Melee Weapon Attack: +5 to   hit, reach 5 ft., one creature. Hit: 5 (1d4 + 3) slashing damage. 
  • Tombstone. Melee Weapon Attack: +5   to hit, reach 5 ft., one target. Hit: 8 (1d10 + 3) bludgeoning damage.

Half-Ghoul

Speed 30 ft., burrow 10 ft.

Human Ancestry. The   half-ghoul is considered both a ghoul and a human. 

Keen Smell. The half-ghoul has advantage on   Wisdom (Perception) checks that rely on smell.

Pack Tactics. The half-ghoul has advantage on an   attack roll against a creature if at least one of the half-ghoul’s allies is   within 5 feet of the creature and the ally isn't incapacitated.

ACTIONS

Leadership (Recharges after a Short or   Long Rest). For 1 minute, the half-ghoul can utter a special command   or warning whenever a nonhostile creature that it can see within 30 feet of   it makes an attack roll or a saving throw. The creature can add a d4 to its   roll provided it can hear and understand the half-ghoul. A creature can   benefit from only one Leadership die at a time. This effect ends if the half-ghoul   is incapacitated.

Multiattack. The half-ghoul makes   one melee attack with its bite and one with its spear.

  • Bite. Melee Weapon Attack: +4 to   hit, reach 5 ft., one creature. Hit: 6 (1d6 + 3) piercing damage. 
  • Spear. Melee or Ranged Weapon   Attack: +5 to hit, reach 5 ft. or range 20/60 ft., one target. Hit: 6 (1d6 +   3) piercing damage, or 8 (1d8 + 3) piercing damage if used with two hands to   make a melee attack.

Priest of Mordiggian

Mordiggian is a Great Old One who sometimes appears as a worm-shaped column of darkness, and others as a limbless, eyeless giant. His cultists cover themselves in funereal purple robes and silver skull masks. They are guardians of the dead, dedicated to ensuring the safety of ghouls and battling necromancers who would defile their temples.

Pack Tactics. As a ghoul. 

Spellcasting. The ghoul is a 10th-level   spellcaster. Its spellcasting ability is Wisdom (spell save DC 14, +6 to hit   with spell attacks). The ghoul has the following cleric spells prepared:

  • Cantrips   (at will): chill   touch, mold earth, thaumaturgy
  • 1st   level (4 slots): arms   of hadar, false life, inflict wounds
  • 2nd   level (3 slots): blindness/deafness,   ray of enfeeblement
  • 3rd   level (3 slots): bestow   curse, speak with dead
  • 4th   level (3 slots): blight,   stone shape
  • 5th   level (2 slots): contagion,   passwall

Dimensional Tunneler. The ghoul can burrow through solid   rock at half its burrowing speed and leaves a 5 foot-wide, 8-foot-high tunnel   in its wake. The ghoul priest can use this power to create a gate to and from   the Dreamlands once a day that lasts for 1 minute. 

Multiattack. The ghoul makes two   melee attacks with its claws.

  • Claws. Melee Weapon Attack: +5 to   hit, reach 5 ft., one creature. Hit: 5 (1d4 + 3) slashing damage. If the   target is a humanoid, it must succeed on a DC 11 Constitution saving throw or   be cursed with ghoul rot. The cursed target can't regain hit points, and its   hit point maximum decreases by 10 (3d6) for every 24 hours that elapse. If   the curse reduces the target's hit point maximum to 0, the target dies, and   its rises as a ghoul. The curse lasts until removed by the remove curse spell   or other magic.
